State and federal regulatory reports about Psychiatric Solutions Inc. facilities – and those operated by its subsidiaries – reveal an array of deficiencies, from holes in the walls to patients who were fatally injured. In a written response to ProPublica, Psychiatric Solutions said the company works to identify problems and change policies to improve operations. âWe sincerely apologize for and regret each incident that has resulted in any harm or distress to one of our patients,â the statement said.
